Guide to the New Student Checklist
What is the New Student Checklist? Why is it important to fill it out? Below is a detailed description of the checklist and why we are asking you to complete each form.
- New Student Housing Form
- Completed by all students to confirm if you plan to live on campus or commute. Questions about the Residential Housing Policy – click here.
- Please have ready a picture of your face showing shoulders and above for your student ID.
- Seminar Preference Form - First year students only
- Please rate the Westminster Seminar topics that sound interesting to you. Then you will be placed into a seminar that fits your interest!
- Complete Language Placement test – First year students only
- All students must complete the Foreign Language placement test.
- Math & English Placement Exam – First year students only
- All students must complete the Math & English Placement Exam questionnaire.
- SOAR Registration Form – First year students only
- Select the date to attend SOAR (Student Orientation, Advising, and Registration). You must complete items 1-4 before you can register for SOAR.
- Parking
- All students need to complete regardless if planning to have a vehicle on campus. Please have vehicle license plate number ready. Click here for questions about parking.
- Emergency Contact Form
- To complete the form, be sure the dropdown shows Fall 2024-2025. Once completed, this form will be processed overnight and have a green checkmark.
- Title IV Authorization Form
- All students must complete whether received federal student air or not. This gives authorization for Federal Student Aid funds to pay for allowable educational related charges.
- Parent Update Form
- Westminster wants to keep in touch with you and your family once classes start. Please make sure we have your most up-to-date personal and parent information.
- FERPA Form
- Once school starts, Westminster can’t speak to anyone except the student about finances, billing, and grades unless the student gives Westminster permission. The only way we can get that permission is through the FERPA form.
- What is FERPA?
- Health Form
- You will need a pdf version of your immunization record to complete the health form.
- *The Health Forms and Immunization Records must be received by the first day of classes.
- Promissory Note Form
- This explains your promise to pay the outstanding balance for your education, forms of communication from the Business Office along with what the delinquent account/collection policy looks like.
